We have your convenience in mind. With the online patient registration portal you can fill out your patient registration any time of day. Registering online will allow us to collect important information that enables us to have a more efficient and productive first meeting. The portal will also allow for better communication and systematic monitoring of your progress during treatment.

Appointments & Cancellations

You may schedule an appointment with a patient care coordinator.

Please be aware that when you make an appointment, that time is your own, as I will not have scheduled anyone else at that time. If you cancel an appointment, I will make every effort to fill that slot with someone on the waiting list; however, this is exceedingly difficult with less than 48 hours of notice. 

Therefore, 48 business hours of notice is required for cancellations or rescheduling for any reason. 

As I am not able to judge the reasons or merits for one being absent, and do not wish to be put in that position, there will be no exceptions to this policy. 

When making a appointment, you are agreeing to accept financial responsibility for your appointment time, whether or not you use it, should you not arrive or arrive late (leaving insufficient time to provide adequate medical care). Likewise, you are agreeing to accept financial responsibility for your appointment time, should you cancel or reschedule within 48 business hours of your appointment.

The fee for such late cancellations is $150.00, which will be charged to the payment method on file.

Emergencies

In the event of an urgency or emergency, do not call or message the office, as we are unable to provide the necessary assistance remotely.  Instead, activate EMS (911) or go to the nearest Emergency Department immediately.  Provide the attending physician with our contact information so we can properly coordinate care.

NOTE: It is not possible to schedule emergency appointments online.

Payment

My office accepts the following methods of payment for co-payments, deductibles and services that are not covered by insurance:

  • Visa
  • MasterCard
  • American Express
  • Discover
  • Zelle

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept MasterCard, Visa, American Express, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them through one of the payment portals in which we participate (Grow, Alma, or Headway). However, payment is ultimately the patient's responsibility. We will help in any way we can to assist you in handling claims.
